Health Information to Be Included in a PHR - Personal Health Record
The Records You Should Plan to Keep in Your PHR

If you decide to put together a PHR (personal health record), find a method that allows you to include the information listed below.

Don't forget to anticipate future needs. For example, if you've never needed an MRI, or if you know that in a few years you'll need the shingles vaccine, then you'll want to be sure your PHR program of choice can accommodate for them when you are ready.

Regardless of whether you use a regular word processing program, a software program purchased for PHR purposes, or an online personal health record keeping system, there are the kinds of information you will want to include:

Your Name

First, middle, last and maiden name, if applicable.

Your Birthdate

Emergency Contacts

Include their contact information, land and cell phones.

Current Date

This reminds you and any professionals who access your information of when your information was last updated.

Health Insurance Information

Include your health insurance company's name and contact information. Include your group number, but not your individual policy number. You'll want to have this available separately. By not keeping all the numbers together, you are taking steps to prevent medical identity theft.

Family History

Include any medical information of blood relatives; history that might genetically affect you. A master list of the important components of a family history will help you record exactly the information your health care provider needs.

Spiritual or Religious Preferences

Should you become very sick, possibly hospitalized, this information could become important. Include the name and contact information of your pastor, priest, rabbi, imam or other spiritual leader.

Advanced Directives

Scanned copies (not the originals) of your advanced directives might include DNRs (do not resuscitate orders), a living will, a healthcare proxy, even information about organ donation.

List of Your Doctors

Include their names, specialties, location, contact information, and identify the health problem you have that they are addressing for you.

Your Last Annual Physical

Including the date of that physical and any tests you were given.
